Skilled Community Nurses are a new NDIS provider currently going through registration. We can currently assist people who are Self-Managed or Plan Managed until our registration is completed and then we can assist everyone with a NDIS Plan.
The Managing Director and Clinical Director have more than 30 years’ experience working in not for profits and the Disability sector and look forward to helping as many people as we can.
The following duties are what our highly trained and regarded community nurses perform. These are specific to the participant’s plan:
✓ Development of care plans
✓ Case management
✓ Medication set-up
✓ Continence advice
✓ Palliative support
✓ Dementia management
✓ Monitoring of blood pressure, pulse, temperature, and blood sugar levels
✓ Diabetes Management
✓ Education for clients and their families
✓ Overnight nursing care
✓ Care after a hospital stay and post-operative care to rehabilitation
✓ Mobility and transfer assistance
✓ Personal care assistance & showering
✓ Support attending appointments
✓ Visiting nurse, general nursing assessment and general well-being checks
